Cinolox contains Ciprofloxacin, a potent second-generation fluoroquinolone antibiotic with broad-spectrum bactericidal activity through inhibition of bacterial DNA gyrase and topoisomerase IV. The 500mg strength represents the most commonly prescribed dose for moderate to severe infections. Ciprofloxacin demonstrates excellent tissue penetration, high oral bioavailability, and activity against both gram-positive and gram-negative bacteria including many resistant strains. However, its use should be reserved for infections where alternative treatments are inappropriate due to serious adverse effect risks including tendon damage, peripheral neuropathy, and CNS effects.

Purpose

Treatment of bacterial infections when alternative treatments inappropriate: urinary tract infections, respiratory infections, skin and soft tissue infections, bone and joint infections, complicated intra-abdominal infections (with metronidazole), infectious diarrhea, typhoid fever, inhalational anthrax post-exposure, plague.

Warnings

BLACK BOX WARNING - Serious adverse reactions: tendinitis and tendon rupture (increased risk >60 years, corticosteroid use, kidney/heart/lung transplant), peripheral neuropathy (potentially irreversible), CNS effects, exacerbation of myasthenia gravis. AVOID FIRST-LINE USE. Reserve for infections with no alternative treatments. Contraindicated with tizanidine, myasthenia gravis. Avoid in children/adolescents (cartilage damage risk). Drug interactions: antacids, iron, calcium, dairy, NSAIDs, theophylline, warfarin. Discontinue immediately if tendon pain, neuropathy symptoms, or serious CNS reactions. Maintain hydration. Photosensitivity - avoid UV exposure. Pregnancy category C. Prescription only.

Side Effects

Nausea, diarrhea, headache, dizziness, rash, abdominal pain, vomiting. SERIOUS: tendon rupture (especially Achilles), peripheral neuropathy, CNS effects (seizures, psychosis), QT prolongation, C. difficile colitis, photosensitivity, hypoglycemia.
