Purpose

Treatment of uncomplicated bacterial infections when alternatives inappropriate: uncomplicated urinary tract infections, acute uncomplicated cystitis in women, uncomplicated gonorrhea (consider resistance patterns), skin and skin structure infections (uncomplicated), infectious diarrhea.

Warnings

BLACK BOX WARNINGS APPLY EVEN AT LOWER DOSES - tendon rupture, peripheral neuropathy, CNS effects, myasthenia gravis exacerbation. Reserve for infections without alternative treatments. Avoid first-line use. Increased risk in elderly (>60), corticosteroid use, transplant recipients. Contraindicated with myasthenia gravis, tizanidine, history of tendon disorders with fluoroquinolones. Separate from multivalent cations (antacids, supplements) by 2-6 hours. Not for children except specific indications. Discontinue immediately if tendon pain, neuropathy, serious CNS symptoms. Hydration essential. Photosensitivity precautions. Pregnancy category C. Prescription only.

Side Effects

Nausea, diarrhea, dizziness, headache, rash, abdominal discomfort. SERIOUS (even at lower doses): tendon rupture, peripheral neuropathy, CNS effects, QT prolongation, phototoxicity, Clostridium difficile-associated diarrhea.
