Co Purpal is a fixed-dose combination of Metformin (biguanide) and Gliclazide (sulfonylurea), providing complementary mechanisms for glycemic control in type 2 diabetes. Metformin improves insulin sensitivity, reduces hepatic glucose production, and delays intestinal glucose absorption without causing hypoglycemia. Gliclazide stimulates pancreatic beta-cell insulin secretion, particularly postprandially. This combination addresses both insulin resistance and secretory defects characteristic of type 2 diabetes, offering superior glycemic control compared to either agent alone while allowing dose reduction of individual components, potentially improving tolerability.

Purpose

Treatment of type 2 diabetes mellitus in adults when diet, exercise, and metformin or gliclazide monotherapy provide inadequate glycemic control. Second-line add-on therapy to improve HbA1c and reduce cardiovascular complications.

Ingredients

Metformin Hydrochloride 500mg, Gliclazide 20mg

Warnings

NOT FOR TYPE 1 DIABETES or diabetic ketoacidosis. Contraindicated in severe renal impairment (eGFR <30), severe hepatic impairment, acute conditions with hypoxia risk, excessive alcohol intake. LACTIC ACIDOSIS RISK with metformin - discontinue before imaging with contrast, surgery, severe illness. HYPOGLYCEMIA RISK with gliclazide - avoid alcohol, monitor blood glucose. Discontinue metformin 48 hours before planned procedures. Vitamin B12 deficiency with long-term metformin. Must be combined with diet and exercise. Regular glucose monitoring and HbA1c testing required. Not during pregnancy or breastfeeding. Prescription medicine.

Side Effects

Hypoglycemia (from gliclazide component), gastrointestinal effects (nausea, diarrhea, abdominal pain, metallic taste from metformin), constipation, vomiting, indigestion, taste changes, weight gain. Rare but serious: lactic acidosis (metformin), severe hypoglycemia.
