Diajard M 12.5/1000mg is dapagliflozin and metformin for type 2 diabetes. Take with meals as directed. Pack of 14 tablets.

Treatment of type 2 diabetes mellitus as an adjunct to diet and exercise.

Dapagliflozin 12.5mg and Metformin hydrochloride 1000mg per tablet.

Contraindicated in severe renal impairment, diabetic ketoacidosis. Not for type 1 diabetes.

Nausea, diarrhea, urinary tract infections, genital mycotic infections.

Store below 25C in a dry place. Protect from moisture. Keep in original pack.
