The central actions of benzodiazepines are mediated through an enhancement of the GABAergic neurotransmission at inhibitory synapeses. In the presence of benzodiazepines, the affinity of the GABA receptor for the neurotransmitter is enhanced through positive allosteric modulation resulting in an increased action of released GABA on the postsynaptic transmembrance chloride ion flux.

Some loss of efficacy to the hypnotic effects of short-acting benzodiazepines may develop after repeated use for a few weeks.
When discontinuing midazolam therapy, insomnia may recur, possibly with a higher severity than before starting treatment (“rebound insomniaâ€).Therefore, it is recommended that the dosage of midazolam is decreased gradually .
It may cause anterograde amnesia, which occurs most frequently within the first few hours after ingesting the product. In order to reduce the risk, patients is advised to ensure that they are able to have an uninterrupted sleep of 7 – 8 hours .
Side Effects
Fluctuations in vital signs including decreased respiratory effects, variation in blood pressure and pulse rate are the most frequently seen findings. Laryngospasm, bronchospasm, dyspnoea, hyperventilation and airway obstruction.Premature ventricular contraction, vasovagal episode and tachycardia.Retrograde amnesia, euphoria, confusion, nervousness, restlessness, nightmares dysphonia and paresthesia.Blurred vision, diplopia, nystagamus, visual disturbances, blocked ears, loss of balance and light headedness.Headache, hiccough, nausea, vomiting, yawning, lethargy, weakness and faint feeling.
