Tapentadol is a centrally-acting synthetic analgesic. The exact mechanism of action is unknown. Although the clinical relevance is unclear, preclinical studies have shown thattapentadol is a mu-opioid receptor (MOR) agonist and a norepinephrine reuptake inhibitor (NRI). Tapentadol exerts its analgesic effects directly without a pharmacologicallyactive metabolite.

Warnings
Misuse, Abuse and Diversion : is associated with abuse liability similar to other opioids: monitor patients closely for signs of misuse, abuse and addiction .
Elderly, cachectic, and debilitated patients and patients with chronic pulmonary disease: Monitor closely because of increased risk of respiratory depression .
Patients with head injury or increased intracranial pressure: Monitor for sedation and respiratory depression. Avoid use of in patients with impaired consciousnessor coma susceptible to intracranial effects of CO2 retention .
Side Effects
The most common (10%) adverse reactions were nausea, dizziness, vomiting and somnolence .
