Rosuvastatin is a competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme that catalyzes the conversion of 3-hydroxy-3-methylglutaryl-coenzyme A to mevalonate, a critical precursor in cholesterol biosynthesis. By decreasing intracellular cholesterol concentrations, it upregulates low-density lipoprotein (LDL) receptors on hepatic cell membranes. This mechanism dramatically accelerates the clearance and catabolism of circulating LDL, lowering total cholesterol, LDL-C, and apolipoprotein B (ApoB). Additionally, it reduces hepatic synthesis of very-low-density lipoprotein (VLDL) and induces a moderate increase in high-density lipoprotein (HDL) levels.

Purpose

It is indicated as an adjunct to diet for the treatment of primary hypercholesterolemia, mixed dyslipidemia, hypertriglyceridemia, and for the primary prevention of major cardiovascular events such as myocardial infarction or stroke.

Warnings

**Myopathy Risk:** Patients must report unexplained muscle pain, tenderness, or weakness immediately, particularly if accompanied by malaise or fever, due to the risk of rhabdomyolysis. **Contraindications:** Strictly contraindicated in patients with active liver disease, unexplained persistent elevations of serum transaminases, and during pregnancy or breastfeeding. **Monitoring Requirements:** Baseline and periodic liver function tests (LFTs) and lipid profiles should be performed during extended treatment regimens.

Side Effects

**Musculoskeletal:** Myalgia, arthralgia, back pain, muscle spasms, and rarely, immune-mediated necrotizing myopathy. **Gastrointestinal & Hepatic:** Nausea, abdominal pain, constipation, flatulence, and asymptomatic elevations in serum transaminases (ALT/AST). **Central Nervous System:** Headache, dizziness, asthenia, insomnia, and mild cognitive impairments.
